New users may not register until their enrollment has been processed by CDA. Plan Participants who provided an email address will receive an Enrollment Confirmation email once their enrollment is complete.

Activating Your New Card

To activate your card at any time, use the automated system by calling the number on the sticker on the front of your CDA card.



If you experience difficulties activating your card from a cellular line, please try the activation process from a landline--the tones are sometimes not properly detected by automated systems when using a cellular line.

 

You may contact CDA, LLC at the phone number on the back of your card if you experience any difficulties activating your card.


Have you Received an Ineligible Transaction Notice?

Your transaction has been paid by you using your card and the payment has been received by the provider/merchant you paid.  After reviewing the documentation submitted to validate your payment, or because requests for documentation have not been responded too, the payment has been determined to be ineligible by the plan. Please take one of the below actions to resolve a denied claim:


  1. Mail a check or money order, payable to CDA LLC, for the amount of the transaction to the above address. Once the repayment is received, the payment will be deposited and credited back to your account balance to be available for future eligible expenses; or
  2. Contact the merchant to let them know the payment was not eligible under your plan and request they refund the amount of the transaction to your Card. Please contact CDA once your merchant has processed the refund so we may attach the refund transaction to satisfy the repayment; or
  3. If you have receipts for other eligible expenses that you have not been reimbursed for and have not used your Card to pay for, you may send those receipts, along with this letter, to CDA and those receipts will be applied to the transaction amount to offset the ineligible balance.

Reporting Your Card Lost or Stolen

Please call CDA immediately at (877)810-2600 to prevent unauthorized transactions on your account.
